No cover sheet is necessary, however, all faxes should contain at least your name and phone number and the person(s) you are sending the fax to (you may wish to call us to confirm receipt of the fax). If you are not a patient of record, please contact the office prior to transmitting a fax.

Office Hours:	8:00am-12:00pm, 1:30pm-5:30pm	We are open every other Monday and Friday, and every Tuesday, Wednesday, and Thursday.

Driving Directions:	Use Mapquest	210 Freeway going WEST Exit Ocean Blvd., turn Left, continue through the second signal (Montrose Ave.), the building will be on the right just past the first side street (Mira Vista). 210 Freeway going EAST Exit Ocean Blvd., turn Right, continue through the signal (Montrose Ave.), the building will be on the right just past the first side street (Mira Vista). 2 Freeway NORTH Stay in the right lane past Mountain Ave., exit Verdugo Blvd., turn Left, proceed to the third stop light (Ocean View Blvd.), turn Right, the building will be on the left, in the middle of the block, just past the first signal (Florencita Ave.).
